Seeking a passionate professional to lead a new small team of Care Coordinators across Lower Hunter and Broken Hill. The team will provide access to Out of Hospital Care packages and supports (ComPacks, SASH, EOL) to assist people obtain the non-clinical after-hospital care they need.Bringing your experience in leading teams in Health, Community Aged Care and/or Disability Support you will use your skills to support your team as well as assist with assessments and case management as required for patients being discharged from Hospital to home. With full rostering support in place and a supportive leadership team, you will be able to spend your time working with your staff and patients/clients.
