The Elements Program at Social Futures works with young people and their families to help strengthen and support relationships. The program aims to provide early intervention and prevention support for young people enhancing family relationships and connection to community. The program works closely with other programs within Social Futures multi-disciplinary suite of services to support the holistic support of the young person we are working with.A core component of this role is to work closely with families to determine their goals and assess their needs. At times this may mean facilitating a warm referral and working collaboratively with other services to meet these needs.
