We work to create positive change in people’s lives and connected, resilient communities.The position forms part of a team within the Connecting Home Specialist Homelessness Service (SHS) comprising of Accommodation and Support, Specialist Complex Case and Youth Housing Support which will be delivered through the Communities North Consortium. The role of the position is to provide a quality, integrated and no wrong door first response to the needs of young people, adults and families, NGO’s, government and the community more broadly.We are seeking a case manager passionate about supporting clients, which in this role is to a large extent via phone based assessment.
