Seeking passionate Home Care and Health professional to be part of a team providing access to Out of Hospital Care packages and supports (ComPacks and SASH) to assist people obtain the non-clinical after-hospital care they need. Bringing your experience in Health, Community Aged Care and/or Disability Support you will use your skills to assess, coordinate supports and provide short-term ongoing case management for patients being discharged from Hospital to home.
